- This form may be used by committee and executive board members only. All expensed over $20 must be approved by the SVBT Treasurer before the purchase is made. A receipt must accompany the form.
- Curriculum Verities Template
- To be considered for a position on the Specialty Committee a CV must be completed. The above CV template may be helpful. Completed CVs may be emailed to Academy of Veterinary Behavior Technicians (AVBT) Julie Shaw, RVT
- Puppy Class Vaccination Recommendation Letter / Canine Behavior Plan of Care
- Two forms on this site: a letter that describes age recommendations for puppies attending puppy socialization classes, and a form to be used by veterinarians who are referring cases to veterinary technicians or trainers for behavior modification.
